Job summary

First Place Pediatrics LLC is seeking a dedicated home health nurse to deliver 1:1 pediatric and, when needed, adult patient care in home settings across California. You will administer medications, monitor progress, and coordinate with care teams to ensure comfort and safety in the patient’s home environment.

The role requires a valid nursing license for California, CPR certification, and prior pediatric home health experience.

Qualifications

• RN license or active nursing degree in the practice state.
• CPR certification required.
• BLS certification preferred.
• Pediatric home health experience preferred.

Responsibilities

• Provide 1:1 patient care.
• Care for pediatric and adult patients with g-tube, trach, and/or ventilator.
• Document care electronically.
• Perform patient assessments.
• Provide in-person home visits.
• Develop meaningful, long-term relationships with patients and families.

Qualifications, Licenses, and Certifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.


• Maintains a current CPR certification
