Job summary

Marin Post Acute is seeking an experienced LVN MDS Coordinator to lead MDS/RAI activities and support reimbursement under PDPM. You will work with an interdisciplinary team, ensure timely documentation, and maintain high standards of resident care.

The role is full-time with competitive pay and benefits, located in San Rafael, CA. Prior SNF/post-acute MDS leadership experience is preferred to drive clinical excellence.

Qualifications

• California LVN license active.
• MDS experience in SNF or post-acute.
• Knowledge of MDS 3.0, RAI, PDPM, Medicare/Medicaid reimbursement.
• Strong assessment and documentation skills.
• Ability to work independently with interdisciplinary teams.

Responsibilities

• Coordinate and manage the MDS/RAI process per CMS, federal, and state regulations.
• Complete accurate and timely MDS assessments and supporting documentation.
• Develop and update comprehensive resident care plans.
• Collaborate with nursing, therapy, dietary, and interdisciplinary team members.
• Monitor PDPM documentation and reimbursement accuracy.
• Track quality measures and identify opportunities for clinical improvement.
• Assist with survey preparation, audits, and regulatory initiatives.
• Educate and support staff regarding MDS processes and documentation requirements.
• Partner with facility leadership to ensure clinical and operational excellence.
