Job summary

Marin Post Acute in San Rafael, CA seeks an experienced LVN MDS Coordinator to lead the MDS/RAI process, ensure regulatory compliance, and optimize reimbursement while collaborating with an interdisciplinary team to improve resident care and outcomes.

Ideal candidates have 1+ year MDS experience in SNF or post-acute settings, active CA LVN license, and strong assessment and documentation skills. This is a full-time position with competitive hourly pay.

Qualifications

• CA LVN license active.
• ≥1 year MDS experience in SNF or post-acute care.
• Knowledge of MDS 3.0, RAI, PDPM, Medicare/Medicaid reimbursement.
• Strong assessment, documentation, and organizational skills.
• Ability to work independently while collaborating with teams.

Responsibilities

• Coordinate and manage the MDS/RAI process per CMS/state regs.
• Complete accurate and timely MDS assessments and docs.
• Develop and update resident care plans.
• Collaborate with nursing, therapy, dietary, and interdisciplinary teams.
• Monitor PDPM documentation and reimbursement accuracy.
• Track quality measures and identify clinical improvement opportunities.
• Assist with survey prep, audits, and compliance initiatives.
• Educate and support staff on MDS processes and documentation.
• Partner with facility leadership for clinical/operational excellence.
