Smith Ranch Skilled Nursing and Rehabilitation, a Generations Healthcare facility in San Rafael, is seeking an LVN MDS Nurse to join our team. You will input MDS data and support PDPM workflows on site, under supervision and within established systems.

Ideal candidates possess a CA LVN license, MDS experience, and the ability to type 50 wpm; MDS 3.0 knowledge and certification are preferred. This is a full-time role with rich benefits and incentives.

Qualifications

• Current and unencumbered CA LVN license.
• 1 year as MDS Coordinator in LTC setting.
• MDS Certified.
• Knowledge of PDPM.
• Typing speed of at least 50 words per minute.

Responsibilities

• Collect and enter MDS data within required time frames.
• Enter MDS information into the MDS system accurately.
• Review and verify data against original MDS documents.
• Lock and file MDS documents in residents' charts.
• Communicate questions or issues to the team to ensure data integrity.
• Maintain patient confidentiality.
