Job summary

PACS is seeking an experienced MDS Nurse (LVN/LPN) for an in-house, on-site role in Long Beach, California. The candidate will coordinate and complete MDS assessments in compliance with federal and state regulations, support reimbursement processes, and collaborate with interdisciplinary teams to promote quality outcomes for residents.

This position requires a current LVN/LPN license, prior long-term care or SNF experience, MDS 3.0/RAI knowledge, CMS/state regulatory compliance, and proficiency

Qualifications

• Current LVN/LPN license in the state of practice.
• Strong knowledge of MDS 3.0/RAI and CMS guidelines.
• Experience in long-term care or SNF settings.

Responsibilities

• Complete MDS assessments and CAAs within required timelines.
• Ensure accuracy of resident documentation for care plans and reimbursement.
• Maintain CMS/state regulatory compliance and coordinate with care teams.
• Collaborate with interdisciplinary teams to develop care plans.
• Monitor progress and update plans; participate in audits and surveys.
• Educate staff on MDS requirements and documentation standards.
