Job summary

Plum Tree Care Center in San Jose, CA is seeking an experienced LVN MDS Coordinator for a part-time role (24 hours/week, in person) at $45-49/hr DOE. The ideal candidate will have prior MDS coordination experience in LTC, CA LVN licensure, and familiarity with MDS 3.0 and PDPM.

Responsibilities include scheduling, entering MDS data into the MDI system, ensuring timely submissions, maintaining chart accuracy, and protecting resident confidentiality. Background check and drug screen are required.

Qualifications

• Current and unencumbered CA LVN license
• 1 year as MDS Coordinator in LTC setting
• MDS Certified preferred
• Knowledge of PDPM
• Proficient in MDS 3.0 reimbursements.
• High School education or GED
• Must have the ability to type a minimum of 50 words per minute

Responsibilities

• Schedules and inputs information from the Minimum Data Set (MDS).
• Enters information from the MDS document into the MDI system by deadlines with accuracy.
• Compares data entered against the original MDS document and makes corrections.
• Retrieves and prints initial feedback reports and final validation reports; locks MDS after final validation.
• Files the MDS documents in residents’ charts.
• Notifies coworkers of questions related to MDS data, delays, system problems, or other obstacles.
• Maintains confidentiality of all patient care information.
