The role requires a California LVN license (RN preferred) with 2 years nursing or 1 year MDS experience, strong communication, and meticulous organization. Duties include staff education, care plan completeness, and collaboration on billing and regulatory reporting.

Responsibilities

• Coordinate and assist with completion and submission of accurate, timely patient assessments.
• Report changes in a patient’s condition to the Director of Nursing (DON).
• Educate direct care staff on patient documentation and regulatory updates.
• Ensure care plans are comprehensive and meet patient needs.
• Review CMS Final Validation Reports to ensure assessments are accepted for billing.
• Collaborate with Business Office regarding billing decisions.
• Review CMS reports to improve accuracy and timeliness of submissions.
• Attend Daily PPS Meeting and Monthly Triple Check.
• Perform staff nurse duties when necessary.
• Lead by example, treating all colleagues with kindness and professionalism.
