Qualifications

• Must be a licensed RN or LVN in the state of practice.
• At least 1 year of home health experience.
• Coding certification is preferred.
• Excellent communication skills, strong organizational abilities, and multitasking capability.
• Must possess a valid state driver’s license and auto liability insurance.

Responsibilities

• Manage schedules for all patients; edit schedules and update agent availability.
• Initiate infection control forms and file infection reports.
• Back-up support during lunch/busy times; assist with case conferences; refer questions to Branch Director.
• Maintain the client hospitalization log; share coordination notes with staff.
• Schedule skilled nurse/injection visits; coordinate add-ons and orders.
• Schedule discharge/OASIS collection or recert visits after case conferences.
• Schedule TIF OASIS collection visits and delete remaining schedules.
• Reschedule declined or missed visits as needed.
• Process reassigned and rescheduled visits; ensure supervisory visits.
• Run scheduling reports (Agent Summary, Missed Visits) and prepare weekly agent schedules.
• Verify visit notes in scheduling console; assist with internal patient transfers between branches.
• If clinical, review lab reports, fax to doctor and copy for Case Manager; route to Medical Records; initiate infection reports.
• If clinical, may perform patient visits or participate in on-call rotation.
