Muir Home Health in the East Bay of California is seeking an experienced LVN Intake Coordinator to join our onsite team in Concord. This role manages the patient intake process, communicates with patients and families, handles data entry, and coordinates insurance verification and authorization.

Formación

• Active California LVN license required.
• 2+ years intake experience in healthcare, home health preferred.
• 2+ years EMR experience, Home Care Home Base preferred.
• Strong communication, negotiation and public relations skills.

Responsabilidades

• Oversee daily patient referral and intake operations with oversight of policies.
• Ensure compliance with state, federal and Joint Commission requirements.
• Coordinate patient education and preparation for home care; initiate plan of care.
• Maximize third-party reimbursement via insurance verification and authorizations.
• Maintain relationships with referral sources and monitor referral statistics.
• Assist in annual budget preparation for the intake department.
• Maintain knowledge of home health contracts and community resources.
