Job summary

Angels Care Home Health is seeking an LVN Intake Coordinator in Texas to coordinate referral intake and care with the interdisciplinary team. The role requires an LVN license, valid driver’s license, and prior home health intake experience where possible.

Qualifications

• LVN license and driver’s license required.
• One year LVN home health experience preferred.
• Experience coordinating intake referrals and using electronic intake systems is a plus.

• Process all aspects of referral intake via Home Care Home Base software system.

Job Functions:

The primary function is to facilitate the intake referral process and coordinate care with interdisciplinary team and the referral source. Will be responsible for processing all aspects of referral intake process via Home Care Home Base software system.

Qualifications:

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as a LVN.
• Current State’s Driver’s License.
• One year experience as an LVN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.
