Job summary

Angels Care Home Health is seeking an LVN Intake Coordinator to manage referral intake and coordinate care with the interdisciplinary team. The primary function is processing all aspects of the referral intake via Home Care Home Base software.

Candidates should be a graduate of an accredited practical/vocational nursing program with an LVN license and a valid driver’s license; one year LVN experience, home health preferred. We offer Monday–Friday schedule and comprehensive benefits.

• Graduate of an accredited school of practical/vocational nursing.
• Current state LVN license.
• Current State’s Driver’s License.
