Job summary

Angels Care Home Health in Abilene, TX is seeking an LVN Intake Coordinator to manage referral intake and coordinate patient care with the interdisciplinary team. The role emphasizes timely processing of referrals via Home Care Home Base and ensuring seamless transitions from referral to care.

The ideal candidate will have at least one year of LVN experience (home health preferred), a valid LVN license, and a valid driver’s license; hepatitis profile proof may be required.

Qualifications

• Graduate of accredited school of practical/vocational nursing.
• Current state license as a LVN.
• Driver's license is required.
• One year LVN experience; home health preferred.
• Proof of hepatitis profile.

Responsibilities

• Facilitate the intake referral process and coordinate care with the interdisciplinary team and referral source.
• Process all aspects of referral intake via Home Care Home Base software.
