Job summary

Angels Care Home Health in Fort Myers is seeking an LPN Intake Coordinator to facilitate referral intake and coordinate care with the interdisciplinary team and referral sources using Home Care Home Base.

The ideal candidate is a graduate of an accredited practical nursing program, holds a current LPN license, a valid driver's license, and has at least one year of home health experience; hepatitis screening is required. We offer benefits and growth opportunities.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• The primary function is to facilitate the intake referral process and coordinate care with the interdisciplinary team and the referral source.
• Will be responsible for processing all aspects of referral intake process via Home Care Home Base software system.
