Job summary

Angels Care Home Health is seeking an LPN Intake Coordinator to support the intake referral process and coordinate care with the interdisciplinary team and referral sources. The role uses Home Care Home Base software to manage referrals and ensure timely processing.

The ideal candidate has an LPN license, a year of LPN experience (home health preferred), and a hepatitis profile. We offer a Monday–Friday schedule with competitive pay and strong benefits.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current State license as an LPN.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• Facilitate the intake referral process.
• Coordinate care with interdisciplinary team and referral source.
• Process all aspects of referral intake via Home Care Home Base software.
