Job summary

Angels Care Home Health is seeking a dedicated LPN Intake Coordinator to streamline the referral intake process and coordinate patient care with the interdisciplinary team. You will manage referrals through the Home Care Home Base system, ensuring timely, compliant processing and communication with referring sources.

The role requires an LPN license, a valid driver's license, and prior home health experience; sales/marketing know-how is a plus.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
• Sales/marketing experience a plus.

Responsibilities

• Facilitate the intake referral process and coordinate care with interdisciplinary team and the referral source.
• Process all aspects of referral intake process via Home Care Home Base software system.
