AltaMed Health Services in El Monte, CA seeks a Licensed Vocational Nurse to develop and implement homecare services for program participants, coordinating all contracted home services, DME, and nursing home care. You will attend scheduled IDT and coordination meetings.

Requirements: CA LVN license, graduation from an accredited LVN program, at least one year of LVN experience, driver’s license with good driving record, and BLS certification.

Qualifications

• Graduate from an accredited LVN program with an active CA LVN license.
• Minimum one year of LVN practice experience.
• Experience with frail or elderly populations preferred.
• Valid CA driver’s license with good driving record.
• Current BLS certification required per AHA/Red Cross.

Responsibilities

Job Overview

Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment, and nursing home care. Attending scheduled IDT and Coordination meetings.

Minimum Requirements

• Graduation from an accredited LVN school, coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required.
• A minimum of one (1) year of experience practicing as a Licensed Vocational Nurse.
• Experience and knowledge regarding the physical, mental, and social needs of the patients are highly desirable.
• At least one year of working with a frail or elderly population
• Valid California driver's license with a good driving record.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

Pay for this job starts at $31.93 hourly
