AltaMed Health Services Corp. seeks a Licensed Vocational Nurse to develop and implement homecare services for program participants, coordinating all contracted home services, durable medical equipment, and nursing home care.

Attendance at IDT and Coordination meetings is required. Requirements include graduation from an accredited LVN program with a CA LVN license, at least 1 year of LVN practice, valid CA driver’s license, and BLS certification or higher.

Qualifications

• Graduation from an accredited LVN school and CA LVN license.
• Minimum of 1 year of LVN practice.
• Valid CA driver’s license with good driving record.

Responsibilities

• Develop and implement homecare services for program participants.
• Coordinate all contracted home services, durable medical equipment, and nursing home care.
• Attend scheduled IDT and Coordination meetings.
