Job summary

St. Joseph Transitional Rehabilitation Center in Las Vegas, NV is seeking a Full-Time Case Manager (LVN/LPN) to lead care coordination from pre-admission through post-admission. You will work with residents, families and an interdisciplinary team to ensure high-quality, person-centered care.

The role requires a graduate LVN program and current unencumbered LVN license in Nevada. You will collaborate across care teams, advocate for residents, and contribute to a supportive, compassionate work

Qualifications

• Must have LVN program graduation and licensure.
• Experience coordinating patient care is preferred.
• Ability to work independently and make quick judgments.

Responsibilities

• Oversee care for residents from pre-admission to post-admission.
• Coordinate services to meet residents' needs in a timely manner.
• Collaborate with physicians, families and interdisciplinary teams to develop and adjust care plans.
• Monitor resident progress and ensure appropriate interventions.
• Facilitate smooth transitions of care and ensure resources are in place.
