Job summary

Villa Las Palmas Healthcare Center is seeking a Case Manager LVN (Full-time) to work with residents, families, and healthcare providers to develop and implement care plans. Qualifications

• Bachelor’s Degree in Nursing or Social Work is required.
• RN license preferred; LVN/LPN acceptable for eligibility.
• Two years of clinical nursing experience preferred.

Responsibilities

• Communicate with residents and families about Plans of Care and treatment protocols.
• Attend morning meetings to coordinate with the care team.
• Perform documentation and chart review in a timely manner.
• Coordinate admissions and discharge activities and referrals.
• Assist with utilization review and cost-effective care planning.
