AltaMed Health Services Corp. is seeking a Licensed Vocational Nurse (LVN) Case Manager in Los Angeles, CA to coordinate care for patients with chronic diseases.

You will conduct case management, coaching, and interventions as part of an interdisciplinary team, including social work and mental health professionals. The role requires an LVN license, at least three years in public health or case management, and willingness to work with diverse patient populations.

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid License as a Licensed Vocational Nurse.
• Three years of experience in public health nursing, case management, or in-home health care.

Responsibilities

• Provide daily care coordination and case management.
• Coordinate with interdisciplinary team including social work and mental health.
• Identify CCS cases and manage transfers and reviews.
