Job summary

AltaMed Health Services Corp. in Los Angeles, CA is seeking a Licensed Vocational Nurse (LVN) Case Manager to provide daily care coordination and case management for patients with chronic diseases.

You will work with an interdisciplinary care team to coordinate services across inpatient and ambulatory settings. Requirements include graduation from an accredited LVN program, current LVN license, BLS certification, and 3+ years in public health or case management; bilingual English/Spanish

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid License as a Licensed Vocational Nurse.
• Experience in and willingness to be part of a multi-disciplinary team.
• Experience with physically or mentally impaired adults and/or the geriatric population.
• Three years of experience in public health nursing, acute care, case management, and/or in-home health care required, minimum of 2 years of managed care experience in case management with a focus on inpatient and/or outpatient ambulatory care preferred.
• Bilingual in English and Spanish preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

• The LVN Case Manager may provide daily care coordination, case management, coaching, consultation, and intervention to patients with one or more chronic diseases.
• May identify population via provider/clinic referral, utilization management referral, disease registry reporting mechanisms, and patient self-referral.
• Coordinate social work and mental health counseling, psycho-social support services, in-home support, legal services, skilled nursing, home health, etc.
• Collaborate with hospitalists, hospital nursing staff, and interdisciplinary care team to ensure seamless transitions of care.
