Job summary

Pleasanton North Nursing and Rehabilitation LSSH Pleasanton I, LLC is seeking an LPN/LVN supervisor to provide direct nursing care and supervise nursing assistants in a long-term care setting. The role ensures compliance with federal, state, and local standards, regulations, and facility policies.

The successful candidate will lead daily nursing operations, manage documentation, participate in quality assurance activities, and coordinate discharge planning to deliver high-quality resident care.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Must possess a current CPR certification.
• Knowledge and experience with PCC preferred.
• Must maintain all required continuing education/licensing.
• Must remain in good standing with the State Board of Nursing at all times.

Responsibilities

• Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
• Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department.
• Participate in facility surveys (inspections) made by authorized government agencies as may be requested.
• Greet residents, supervise nursing care, and ensure adherence to care plans.
• Oversee nursing care, documentation, and discharge planning to maintain the resident's well-being.
