• Serve as primary liaison between the facility and hospital discharge planners
• Coordinate with referral sources to identify unmet healthcare needs and cultivate relationships
• Complete admissions paperwork and contracts for residents
• Lead admissions and census meetings to drive business growth
• Maintain marketing materials and information packets for referrals
• Prepare daily census and monthly/quarterly reports as assigned
• Screen potential admissions for payer source, authorization, Medicare eligibility, and level of care
• Process admissions including notifying nursing staff and providing resident charts
• Maintain relationships with referral sources and families post-admission
