Job summary

The Meadows Post Acute in Panorama City, CA is seeking an LVN Admissions Director to lead admissions and coordinate with hospital partners. You will manage the referral process, verify payer sources, and ensure timely, compliant admission workflows.

You will collaborate with clinical and administrative teams, generate census reports, and maintain strong community relations to support growth and quality resident care.

Qualifications

• LVN license is required.
• Previous admission experience in SNF is required.
• Strong interpersonal and organizational abilities.

• Solid knowledge of reimbursement processes.

• Familiarity with medical terminology preferred.

Responsibilities

• Assist in developing and implementing facility marketing strategies to attract new residents.
• Serve as the primary contact between the facility and hospital discharge planners to ensure seamless transitions.
• Communicate with referral sources to build relationships and identify unmet healthcare needs in the community.
• Complete necessary paperwork to ensure appropriate admissions to the facility.
• Lead team meetings regarding admissions and census to drive business growth.
• Order and maintain adequate marketing materials and ensure information packets are available.
• Prepare daily census reports and complete monthly and quarterly reports as assigned.
• Handle inquiries from hospitals and the community.
• Screen potential admissions, including payer source, authorization, Medicare eligibility, and level of care.
• Process admissions, including notifying nursing staff, creating resident IDs, providing charts, and completing admission contracts.
• Maintain public relations with referral sources and foster ongoing internal public relations with residents and families post-admission.

Salary:

$34 - $40 HOURLY
