Job summary

Methodist Hospital in San Antonio is seeking a Nurse anchored Post-Transplant Coordinator to assess, plan, implement, and evaluate care for patients with chronic organ failure, end-stage disease, LVAD support, and transplantation.

As part of the transplant team you will coordinate care across clinicians to achieve positive patient outcomes and support families during complex postoperative periods.

Qualifications

• Basic Cardiac Life Support must be obtained within 30 days of employment start date.
• RN (Registered Nurse) required.
• Registered Nurse Diploma required.
• LVAD experience preferred.
• Minimum 2 years of critical care experience in hospital setting required.

Responsibilities

• Assess, plan, implement, and evaluate care for patients with chronic organ failure, end-stage organ disease, LVAD and transplant patients.
• Coordinate care delivered by other transplant team members to affect positive outcomes for the patient.
• Support the transplant team in coordinating post-transplant care and LVAD-related management.
