Job summary

HCA Healthcare's Methodist Hospital is seeking an RN Transplant Coordinator for the Lung Clinic to assess needs, educate patients and coordinate care throughout the transplantation process. You will work with patients, families, and the multi-disciplinary team to ensure a seamless plan of care from pre-admission through follow-up care.

Responsibilities include coordinating pre-admission workups, collaborating with providers, and communicating with payers to optimize outcomes for transplant

Qualifications

• Bachelors degree in nursing preferred.
• Three years of recent clinical experience preferred.
• Texas RN license required.

Responsibilities

• Assess patient and family needs and coordinate care through transplant process.
• Educate patients and families about disease, procedures, and care plans.
• Coordinate pre-admission workups and donor/patient visits with the transplant team.
• Ensure continuity of care across departments and with home health providers.
• Communicate plan of care with patients, families, other providers, and payers.
