Advocate Health’s Lung Nodule Program in Charlotte, NC seeks an experienced Registered Nurse (RN) to serve as a nurse navigator for patients with lung nodules. The role combines remote and in-person duties, requiring strong communication, care coordination, and patient education to support multidisciplinary teams.

The position is full-time with a hybrid schedule, travel to ambulatory practices for education and outreach, and a focus on timely follow-up and pathway optimization.

Competenze

• Completion of an accredited nursing program.
• Current Registered Nurse license in the practicing state.
• Current BLS certification.
• Minimum of one year of nursing experience preferred.
• Pulmonary/oncology/ambulatory care experience preferred.

Mansioni

• Serve as nurse navigator for patients with identified lung nodules.
• Assess clinical information and imaging to determine next steps.
• Perform clinical triage, risk stratification, and symptom escalation.
• Provide patient outreach, education, and support by phone, virtually, and in person.
• Coordinate pulmonary access, appointments, and multidisciplinary review.
• Identify barriers to care and monitor patients through pathways.
• Support standardized workflows and quality-improvement efforts.
