Job summary

UCSF is hiring a Clinical Nurse Navigator for the Lung Nodule Outpatient Clinic. The CNII leads care coordination for an adult oncology population, guiding patients through referrals and services with a focus on safety and excellence in nursing practice.

Role emphasizes leadership within interdisciplinary teams, adherence to oncology standards, and collaboration with physicians, dietitians, social work, and financial services to support patient care throughout the referral process.

Qualifications

• Five or more years of recent RN adult ambulatory care experience.
• A minimum of 3 years of total RN experience.
• Experience with Lung Nodule / Thoracic Surgery adult population within the last year.
• Currently licensed as a Registered Nurse in California.
• Current healthcare provider CPR certification.

Responsibilities

• Coordinate patient care for an assigned adult Oncology outpatient population.
• Provide care coordination for patients referred to the Lung Nodule Clinic.
• Assess and match unmet needs with appropriate services and referrals.
• Utilize the nursing process for assessment, planning, implementation and evaluation.
