ThedaCare is seeking a Lung Nodule Coordinator to manage patients with incidental or screening-detected nodules, coordinating care with Cardiothoracic Surgery, Pulmonology, Radiology, Oncology, and Primary Care to ensure timely evaluation and evidence-based follow-up.

Responsibilities include tracking imaging, educating patients, arranging referrals, and maintaining registries while guiding patients through the diagnostic pathway and ensuring no loss to follow-up.

Qualifications

• Associate degree in Nursing, Respiratory Therapy, Radiologic Technology, or another healthcare-related field.
• Minimum of 2 years RN, RT, Imaging or Care coordination experience
• Preferred Clinical background in pulmonology, oncology, cardiothoracic surgery, or care navigation
• American Heart Association Basic Life Support (BLS) or CPR certification for healthcare providers

Responsabilités

• Coordinate evaluation and follow-up of patients with pulmonary nodules through incidental findings, lung cancer screening, or diagnostic imaging.
• Serve as the primary point of contact for patients during the diagnostic evaluation period.
• Review imaging reports and identify patients requiring follow-up according to guidelines and protocols.
• Facilitate timely referrals to Cardiothoracic Surgery, Pulmonology, Interventional Pulmonology, Radiology, and other specialists.
• Coordinate diagnostic testing (repeat imaging, PET, PFTs, biopsies, specialty consultations).
• Track patients through completion of diagnostic pathways and ensure care-gap closure.
