Job summary

Advocate Health is seeking a dedicated Registered Nurse to join the Lung Nodule Program as a nurse navigator. The role blends remote and on-site responsibilities at the Kenilworth Medical Office Building in Charlotte, NC, with periodic travel to ambulatory practices for education and program support.

Ideal candidates bring strong organization, adaptability, and collaboration skills, with pulmonary care, oncology, or care coordination experience preferred.

Qualifications

• Completion of an accredited nursing program.
• Current Registered Nurse license issued by the state.
• Current BLS certification.
• Minimum of one year nursing experience preferred.
• Pulmonary, oncology, nurse navigation, ambulatory care, or care coordination experience preferred.

Responsibilities

• Serve as nurse navigator for patients with lung nodules.
• Perform clinical triage, risk stratification, and symptom escalation.
• Provide patient outreach, education, and support by telephone, virtually, and in person.
• Communicate with patients, referring providers, and interdisciplinary care teams.
• Coordinate pulmonary access, appointments, diagnostic follow-up, and multidisciplinary review when indicated.
• Identify and help resolve barriers to care while monitoring patients through the recommended pathway.
• Support standardized workflows, tracking processes, and quality-improvement efforts that promote timely follow-up and reduce gaps in care.
• Provide education and outreach to ambulatory practices regarding the lung nodule pathway.
