Job summary

University of California - San Francisco is seeking a Clinical Nurse Navigator in the Lung Nodule Outpatient Clinic. Under general supervision, the CN coordinates patient care for referred adults, guiding them through the referral process and linking to services such as dietitians, social work, and financial services.

The CNII uses the nursing process to provide safe, therapeutic care and collaborates with interdisciplinary teams.

Qualifications

• Three (3) or more years of recent RN adult ambulatory care experience.
• A minimum of 3 years of total RN experience.
• This position requires flexibility to orient and work at all UCSF Medical Center locations.
• Three (3) or more years of recent RN oncology experience (preferred).
• OCN or other relevant specialty certification (preferred).
• Current licensure as a Registered Nurse in the State of California.
• Current healthcare provider CPR certification.

Responsibilities

• Identify the functions or tasks that employees in the job perform. The essential functions should state the purpose of the work and the results to be accomplished, rather than how the function is performed.
• Coordinates all new patients by conducting phone intake, triaging according to practice guidelines, and ensuring information is obtained prior to new patient appointments.
• Provides/coordinates ongoing care for established patients by performing nursing assessments and managing care plans.
• Facilitates communication between providers and attends tumor board and staff meetings.
• Acts as a referral liaison with oncology staff to coordinate care among providers and community agencies.
• Prevents delays in treatment by identifying and addressing problems in care.
• Works with Cancer Registry to collect data and support planning.
• Identifies resources for patients and families and helps them access services.
• Develops efficient procedures to improve patient flow and education.
• Supports care team in plan of care formation.
