Job summary

Advocate Health’s Lung Nodule Program is seeking a dedicated Registered Nurse to join a multidisciplinary team. This hybrid role combines remote responsibilities with in-person work at the Kenilworth Medical Office Building, with periodic travel to ambulatory practices for education, outreach, and program support.

The ideal candidate has an accredited nursing program, an active RN license, and BLS certification, with 1+ year of nursing experience; pulmonary/oncology or nurse navigation

Qualifications

• Completion of an accredited nursing program.
• Current Registered Nurse license issued by the state.
• Current BLS certification.
• Minimum of one year of nursing experience preferred.
• Pulmonary, oncology, nurse navigation, ambulatory care, or care coordination experience preferred.

Responsibilities

• Assess clinical information and imaging recommendations to help determine appropriate next steps.
• Perform clinical triage, risk stratification, and symptom escalation.
• Provide patient outreach, education, and support by telephone, virtually, and in person as appropriate.
• Communicate with patients, referring providers, and interdisciplinary care teams.
• Coordinate pulmonary access, appointments, diagnostic follow-up, and multidisciplinary review when indicated.
• Identify and help resolve barriers to care while monitoring patients through the recommended pathway.
• Support standardized workflows, tracking processes, and quality-improvement efforts that promote timely follow-up and reduce gaps in care.
• Provide education and outreach to ambulatory practices regarding the lung nodule pathway.
