Qualifications

• Valid driver’s license and reliable transportation for travel.

Responsibilities

• Conduct in-home, face-to-face assessments for Medicaid members.
• Develop and monitor care plans and waiver services.
• Coordinate with multidisciplinary team to ensure quality, cost-effective member care.
• Facilitate waiver enrollment/disenrollment and ICT meetings as needed.
• Travel 25–40% locally for member visits.
