Job summary

UnitedHealthcare seeks a Case Manager for LTSS to conduct intake, assessments and ongoing care planning for members in Colorado. You will travel to member homes or facilities, coordinate services, and maintain up-to-date care plans in the system.

The role requires 50% travel, strong communication, confidentiality, and the ability to work hybrid from the Delta, CO area. Local residency and transportation are essential.

Qualifications

• Bachelor's degree in human behavioral sciences or related LTSS field.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Ability to travel up to 50% within the assigned territory.
• Local residence near Delta, Hinsdale, or Gunnison County, CO.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of the member's health and welfare, documentation in the prescribed system.
• Maintain confidentiality of patient information in accordance with the Employee Handbook.
• Conduct regular monitoring contacts per LTSS program rules; update assessments and care plans accordingly.
• Face-to-face contact with members at least every six months; adjust documentation as needed.
• Complete annual reassessments and any required additional assessments.
• Monitor service delivery per the Support Plan and PAR; participate in CMA trainings.

Education

Bachelor's degree in human behavioral sciences (human services/social work/psychology/public health)

5+ years LTSS experience
