Job summary

Centene Corporation is seeking a Care Manager in Polk County, IA. This hybrid role involves four days of in-person member visits per week within Polk County, with remote work flexibility when not conducting field visits.

You will develop and monitor long-term care plans, educate members and families, coordinate with providers, and ensure compliant record-keeping. Requires a Bachelor's degree and 1 year of related experience (RN with 6+ years in population served noted for Iowa plan).

Responsibilities

• Evaluates member needs and coordinates care plans for best outcomes.
• Develops long-term care plans and identifies providers and community resources.
• Facilitates communication between member/family and care team for service access.
• Provides education to members and families on procedures, services, and benefits.
• Monitors plans, status, and outcomes to make recommendations for improvement.
• Performs on-site visits and collaborates with providers as needed.
• Ensures accuracy and compliance in member records and care management activities.
