Centene is seeking a field-based Care Coordinator in Polk County, IA. This hybrid role combines in-home member visits four days per week with flexible remote work when not conducting field visits.

• Bachelor's degree and 1 year related experience required.
• For Iowa Plan Only: Bachelor's degree required and 1+ years of experience with populations served, or RN with 6+ years of experience with population served.

Responsabilidades

• Evaluates the needs of the member, resources available, and recommends the plan for the best outcome.
• Assists with developing ongoing long-term care plans and identifies providers and community resources.
• Coordinates between member/family/caregivers and care team to ensure services are accessible.
• Provides resource support to members and families on employment, housing, and other needs based on assessments.
• Monitors care plans and outcomes, providing recommendations based on identified needs.
• Interacts with long-term care providers to ensure member needs are met.
• Collects and maintains member information to ensure regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Provides education to members and caregivers on procedures and benefits.
• Gives feedback to leadership on improving quality of care and cost-effectiveness.
