Centene Corp. is seeking a LTSS Service Care Coordinator for a hybrid role based in Woodbury County, IA, with in-person member visits four days a week.

Responsibilities include evaluating member needs, coordinating long-term care plans, and facilitating access to providers and community resources. You will educate members and families about services and benefits, monitor progress, and collaborate with care teams to ensure quality, cost-effective care.

Formación

• Requires a Bachelor's degree and 1 year of related experience.
• Or equivalent experience reflecting the level of this position.

Responsabilidades

• Assist in developing and coordinating long-term care plans and service plans.
• Coordinate between member, family/caregivers and care providers to ensure access to services.
• Educate members and families on procedures, services, options and benefits.
• Monitor care plans, member status and outcomes, and provide recommendations to improve care.
