Job summary

Centene Corp. seeks a Service Care Coordinator in Des Moines, IA, to develop and coordinate long-term care plans for members in their homes and communities. The role blends in-person visits four days a week with remote work on other days, across Polk County.

You will evaluate member needs, connect with providers and resources, educate families, and monitor outcomes to improve quality and cost-effective care. A Bachelor’s degree and related experience are required.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• For Iowa Plan Only: Bachelor's degree required and 1+ years of experience with populations served, or RN with 6+ years of experience with population served.

Responsibilities

• Evaluates the needs of the member, resources available, and recommends or facilitates the plan for the best outcome.
• Assists with developing ongoing long-term care plans/service plans and identifies providers, specialists, and community resources needed for long-term care.
• Coordinates between the member and family/caregivers and the care provider team to ensure services are accessible.
• Provides resource support to members and families/caregivers on procedures, options, referrals, and benefits.
• Monitors care plans, member status and outcomes, and provides recommendations to improve quality and cost-effectiveness.
