Job summary

Centene is seeking a LTSS Service Care Coordinator to assist in developing, assessing, and coordinating holistic care management activities for quality, cost-effective outcomes in long-term care settings.

The role emphasizes evaluating needs, coordinating with providers, and educating members and families on benefits, with on-site visits and ongoing plan development. Bachelor’s degree is required, with RN licensure for Iowa Plan eligibility, and 1+ years of related experience preferred.

Qualifications

• Bachelor's degree is required.
• 1+ years of related experience, or RN with 6+ years of experience with the population served.
• RN - State Licensure required for Iowa Plan.

Responsibilities

• Evaluate the needs of the member and recommend/facilitate plans for the best outcome.
• Assist with developing ongoing long-term care/service plans and identify necessary providers and resources.
• Coordinate between the member/family and care provider team to ensure accessibility of services.
• Provide resource support for various needs based on service assessment and plans.
• Monitor care plans and member status, providing recommendations based on identified needs.
• Interact with long-term care healthcare providers to ensure member needs are met.
• Collect, document, and maintain member information to ensure compliance with regulations.
• Perform on-site visits to assess member needs and collaborate with providers as appropriate.
• Facilitate education for long-term care members and their families on healthcare options and benefits.
• Provide feedback to leadership on opportunities to improve quality of care and service delivery.
