Assists in developing long-term care plans and coordinates with providers and resources.
Coordinates between member/family and care provider team to ensure access to services.
Provides resource support for employment, housing, independent living, and related needs.
Monitors care plans and outcomes and makes recommendations based on member needs.
Interacts with long-term care providers and partners to meet member needs.
Collects and documents long-term care information to ensure regulatory compliance.
May perform on-site visits to assess needs and collaborate with providers.
Provides education to members and families on procedures, options, and benefits.
Offers feedback to leadership to improve quality and cost-effective care delivery.
