Qualifications

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Develop and implement care plans with members, caregivers, physicians and other professionals, addressing member needs and goals.
• Monitor care plan effectiveness, document interventions and adjust as needed.
• Promote integration of LTSS and behavioral health services with home/community resources.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for approval/denial of services and collaboration.
• Educate and motivate change during member contacts using motivational interviewing.
• Identify barriers to care and assist with psycho/social, financial, and medical obstacles.
• Identify critical incidents and develop prevention plans for member welfare.
• Travel approximately 25-40% as required by state/contractual requirements.
