Job summary

Molina Healthcare is seeking a Care Coordinator to support care management and LTSS-related activities. You will collaborate with a multidisciplinary team to coordinate integrated member care across settings and ensure progress toward outcomes while contributing to the broader strategy for quality and cost-effective care.

Essential duties include completing member assessments, facilitating waiver enrollment, developing care plans, monitoring progress, and promoting service integration.

Qualifications

• At least 2 years of health care experience, including LTSS exposure.
• Clinical licensure and/or certification only if required by state contract or regulation.
• Bachelor's degree in a health care-related field may be required in some states.
• Valid driver's license and reliable transportation for job-related travel.
• Knowledge of community resources; ability to work with diverse populations.
• Proficiency with Microsoft Office and navigating online portals.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care team.
• Monitor care plan effectiveness and document interventions and goal achievement.
• Promote integrated services for members, including behavioral health care and LTSS.
• Assess medical necessity and authorize waiver services.
