Molina Healthcare is seeking a Care Manager to support LTSS activities and coordinate member care with a multidisciplinary team to achieve quality outcomes across the care continuum. You will conduct comprehensive assessments, develop and monitor care plans, and promote service integration with behavioral health and community resources.

The role requires 2+ years of healthcare experience, a valid driver’s license, strong communication, and the ability to work independently across diverse

Competenze

• 2+ years of health care experience, including LTSS exposure.
• 1 year in care management or equivalent setting.
• Licensure/certification required only if state contract mandates it; active/unrestricted if licensed.
• Bachelor's degree may be required in some states.
• Valid driver's license and reliable transportation for travel.
• Demonstrated knowledge of community resources.
• Ability to work in diverse settings and with varying personalities.
• Strong communication, organization, and problem-solving skills.
• Proficiency with Microsoft Office and online portals.

Mansioni

• Completes comprehensive member assessments within regulated timelines, including in-person home visits when required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with member, caregivers, physicians, and care professionals.
• Monitors care plan effectiveness and documents progress.
• Promotes integration of services including behavioral health and LTSS.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and advises on funding sources.
• Facilitates interdisciplinary care team meetings for approvals/denials.
• Uses motivational interviewing and Molina clinical guidelines to support change.
• Identifies barriers to care and coordinates psycho-social, financial, and medical support.
• Identify critical incidents and develop prevention plans.
• Travel 25-40% as required by state/state-contract requirements.
