Job summary

Molina Healthcare is seeking a Care Manager to support LTSS/care coordination and collaborate with a multidisciplinary team to deliver integrated member care. You will assess needs, develop plans, and monitor progress toward outcomes while facilitating waiver processes and ensuring service quality.

Ideal candidates have 2+ years of healthcare experience, LTSS or care management exposure, and a valid driver’s license for travel to member homes and community sites.

Qualifications

• 2+ years healthcare experience including LTSS/care management or equivalent.
• Licensure/certification required only if state contract mandates it.
• Bachelor's degree may be required by state/contract.
• Valid unrestricted driver’s license and reliable transportation for travel requirements.
• Experience with community resources and working with diverse populations.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers and providers.
• Monitor and adjust care plans based on progress and barriers.
• Coordinate LTSS and behavioral health services to ensure continuity of care.
• Assess medical necessity and authorize appropriate waiver services.
• Coordinate ICT meetings for service approvals/denials.
• Travel 25-40% as required by state/contractual requirements.
