Job summary

Molina Healthcare, Inc. seeks a care management professional to support LTSS activities and collaborate with a multidisciplinary team to deliver integrated member care.

You will perform comprehensive assessments, develop and monitor care plans, and facilitate waiver services to meet member needs and goals. Responsibilities include coordinating care with physicians, caregivers, and healthcare professionals, ensuring regulatory timelines, and promoting access to community resources.

Qualifications

• At least 2 years of healthcare experience with LTSS preferred.
• Demonstrated knowledge of community resources and care coordination.
• Strong verbal and written communication to work with diverse populations.

Responsibilities

• Completes comprehensive member assessments within regulated timelines.
• Facilitates waiver enrollment and disenrollment processes.
• Develops care plans with members, caregivers, physicians, and others.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Coordinates with interdisciplinary care teams for approvals/denials.
• Travel 25-40% may be required.
