Job summary

Molina Healthcare, Inc. is seeking a Care Manager to support LTSS care coordination across the continuum for members with high-need potential.

You will collaborate with a multidisciplinary team to ensure progress toward outcomes and to deliver integrated, quality member care. The role involves completing comprehensive assessments, coordinating waiver enrollment, developing care plans with providers and families, monitoring progress, and advocating for access to services.

Qualifications

• 2+ years health care experience, LTSS exposure preferred.
• Clinical licensure and/or certification if required by contract/regulation.
• Bachelor's degree in a health care field may be required by state.
• Valid driver’s license and reliable transportation for travel.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person visits when required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and support networks.
• Monitor care plans to evaluate effectiveness and adjust as needed.
• Promote integration of LTSS and behavioral health services to ensure continuity of care.
• Assess medical necessity and authorize appropriate waiver services.
• Collaborate with interdisciplinary care teams for service approvals and collaboration.
• Utilize motivational interviewing and Molina guidelines to educate and support change.
• Identify barriers to care and assist with psycho-social, financial, and medical obstacles.
