Qualifications

• Minimum 2 years health care, incl care management and LTSS.
• Licensure as LCSW/LMSW/APSW or equivalent; active in state.
• Valid driver’s license and reliable auto insurance; travel 25–40%.
• Ability to work independently and with diverse populations.
• Strong communication and documentation skills.

Responsibilities

• Complete comprehensive member assessments within timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with care team and member/vendor collaboration.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of LTSS and behavioral health services.
• Assess medical necessity and authorize waiver services.
• Lead ICT meetings for service approvals.
• Use motivational interviewing to support member changes.
