Qualifications

• At least 2 years of experience in health care including 1 year in care management or LTSS.
• Licensed clinical social worker or equivalent licensure in state of practice.
• Valid driver's license and ability to travel 25-40% as required.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including required in-person visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care teams.
• Monitor and adjust care plans to measure outcomes and effectiveness.
• Promote integration of LTSS and behavioral health services to ensure continuity of care.
