Job summary

Molina Healthcare is seeking a Care Manager to support LTSS-related activities and collaborate with a multidisciplinary team to deliver integrated member care. The role focuses on assessments, care planning, and case coordination to achieve quality outcomes while managing costs.

The position requires at least 2 years in health care with LTSS experience, strong communication, and ability to travel as needed. A valid driver's license is preferred; competitive benefits are offered.

Qualifications

• 2+ years health care experience, LTSS exposure and care management experience.
• Clinical license and/or certification required only if state contract mandates it.
• Bachelor's degree may be required in some states.
• Valid driver’s license and reliable transportation for travel.
• Experience with diverse populations and ability to work independently.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team to address needs and goals.
• Monitor care plans; document interventions and progress toward goals.
• Promote integration of services across behavioral health and LTSS.
• Assess for medical necessity and authorize waiver services as appropriate.
• Coordinate interdisciplinary care team meetings for approvals/denials.
