Qualifications

• Strong communication and documentation skills.

Responsibilities

• Conduct comprehensive member assessments within required timelines.
• Coordinate waiver enrollment/disenrollment and care plans with teams and caregivers.
• Monitor care plans and adjust interventions to meet goals.
• Collaborate with ICT meetings for service approvals/denials.
• Travel 25-40% to member homes as needed.
